These screens come from a synthetic case: a 196-page record set for an invented patient after a rear-end collision. Every name, date, and facility is made up, and no real patient records were used.

Every billing line is tied to its statement page. Possible duplicate statements and repeated same-day charges are flagged for the nurse to decide, not silently added or dropped.

The nurse confirms the document was read to the end, then signs. The export carries the signature and the nurse’s own branding, as PDF and Excel.
